Transcript

 

The Registrar's Office issues transcripts for students as requested when the student uses their electronic access via myPHCC. Students should be sure to include a complete address for where they want the transcript sent. For students who do not have electronic access, a transcript will be issued upon signed, written request to the Office of the Registrar. Mailing address: 645 Patriot Ave, Martinsville, VA 24112.  The signed, written request may also be sent by fax to (276)656-0352.
